Our Privacy Policy and Terms And Conditions are the result of changes in data protection law and therefore provide transparency about how we collect, process and store information about you, our patient.

This Privacy Policy and these Terms Of Conditions (which we encourage you to read) will take effect on 25 May 2018. 

As of this date, we will ask you for your written permission to use your data for osteopathy and the associated processing activities.

We also ask you for your permission for the mandatory use of your social security number.

What do we use your personal information for?

We use your personal data for being able to properly perform the requested osteopathic services, to send you our newsletter and to be able to call or email you to schedule appointments for osteopathic services. You have the possibility to open your own account.

We, osteopathy Verstraten, take safeguards to protect your privacy. We ensure that our employees only get access to your data to the extent necessary for the performance of their function.

With whom do we share data

If you wish to contact another healthcare provider, lawyer or insurer, the osteopath will be obliged to use your citizen service number in that correspondence with other healthcare providers about the health data of you as a patient. You agree that in these situations medical information is shared with other care providers.

Furthermore
we engage with third parties for the IT and administrative services. These third parties process the data only for our benefit and for no other purpose. We have made agreements with these parties about the processing of all personal data like their security and data retention. These agreements are stored in the register of processing activities through contracts concluded with these companies. This is how we save and protect your sensitive privacy and health data.

You have the right to see, to correct and delete the personal settings of your account.  You can do this on your own via “persoonlijke instellingen” of your account. In addition, you have the right to withdraw your possible consent or object to the way your personal data is being processed by our company data and you have the right to transfer your data.

This means that you can submit a request to us to send the personal data that we store about you in a computer file to you or another organization of your choice.

To ensure that the request for the data access has been made by you, we request to send a copy of your passport or ID card. Make your passport photo in this copy, MRZ (machine readable zone: the stripe with numbers at the bottom of the passport) passport number and BSN number are visible. Osteopathy Verstraten will respond to your request as quickly as possible and certainly within 4 weeks.

How long do we keep your data

The WGBO Act states that medical records must be kept for 15 years.
